LAKEWOOD — Chautauqua Hospice & Palliative Care (CHPC) invites golfers and community members to participate in the Hole Out for Hospice Golf Tournament, scheduled for Aug. 9 at Chautauqua Golf Club. This event offers a unique opportunity to honor the memory of loved ones while supporting a vital cause.
The event is made possible thanks to the generous support of Howard Hanna Holt Real Estate. “We’re excited to partner with Howard Hanna Holt and spend a great day with them on the course. We are grateful for their support,” said Zach Agett, vice president of community engagement at CHPC.
Participants can sponsor a memorial sign to create a lasting tribute to cherished individuals while helping CHPC provide compassionate care to patients and families facing serious illness. These signs will be prominently displayed on the golf course throughout the day.
Registration for the Hole Out for Hospice Golf Tournament includes 18 holes of golf, a cart, entry into the 50/50 raffle, a gift bag, a light breakfast, food on the turn, and a light dinner.

Whether through financial support, products, or services, charitable contribution allows CHPC to continue offering the highest level of compassionate care, vital education and comfort to those facing challenging times. Even the smallest donations make a significant impact.
Memorial signs must be reserved by July 29. For more information, visit chpc.care/golf or contact Agett at zacharya@chpc.care. Together, we can make a lasting impact on our community.
