CHAUTAUQUA COUNTY SHERIFF’S OFFICE
¯ FORESTVILLE — Jaralyn E. Majak, 30, of Forestville was charged with driving while intoxicated, DWI per se, speeding and unlicensed operator. Deputies on March 13 located a vehicle speeding above the posted limit. Majak was taken into custody following an investigation. Majak will answer the charges in Sheridan Town Court at a later date.
¯ SHERIDAN — Jose G. Carabello, 28, of Forestville was charged with driving while intoxicated, DWI per se and speed in zone. Deputies observed a vehicle speeding in Sheridan on March 15 at about 11:30 p.m. Carabello was taken into custody following an investigation. Carabello will answer the charges in Sheridan Town Court at a later date.
¯ Jessica A. Corkins, 42, of Forestville was charged with third-degree welfare fraud, seven counts of first-degree offering a false instrument for filing, misuse of food stamps and third-degree grand larceny. From Sept. 1, 2014, to Sept. 30, 2017, Corkins allegedly ailed to notify the Chautauqua County Department of Health & Human Services that her children’s father had income/employment while residing with Corkins. This unreported income allowed for Corkins to receive $10,822.00 in unentitled benefits. She was issued appearance tickets to be in Jamestown City Court at a later date.

¯ Grant H. Monroe Jr., 42, was charged with felony driving while intoxicated, aggravated DWI, fourth-degree criminal mischief, aggravated family offense, unlawful imprisonment and criminal obstruction of breathing. Officers were conducting an investigation into a domestic violence incident that took place Wednesday on Sampson Street, near McKinley Avenue. After speaking to a female victim, officers determined that a physical altercation had taken place at the residence in addition to a TV being damaged. Monroe returned the residence in a vehicle, and an investigation found that he had been intoxicated. He was transported to Jamestown City Jail and held for arraignment.
¯ Hector L. Rivera, 41, of Jamestown was charged with seventh-degree criminal possession of a controlled substance. Officers checked with the well-being of a male Wednesday at 10:16 p.m. in the area of 50 W. Harrison St. Upon speaking to Rivera, officers observed several bags hanging from his pocket. A check found several bags allegedly containing a white/brown powdery substance, later identified as heroin. Rivera was held in Jamestown City Jail for arraignment.
