CHAUTAUQUA COUNTY SHERIFF’S OFFICE
POMFRET-Jesse D. Huddleston, 31, of Warren, Pa., was charged Thursday with failing to keep right, third-degree aggravated unlicensed operation and fourth-degree criminal possession of a weapon. At approximately 6:49 p.m., deputies responded to a property damage car accident on Route 60 in Pomfret. After a brief investigation, it was determined that one of the drivers-Huddleston-had a suspended driver’s license. Huddleston was also allegedly in possession of a collapsible baton. Moreover, he had an outstanding warrant from Lakewood-Busti Police Department and was turned over to th Lakewood-Busti Police Department.
VILLENOVA-Jane Schimek, 20, of Cherry Creek was transported to Lakeshore Hospital Friday after sustaining injuries in a car accident on South Dayton Silver Creek Road in Villenova. At approximately 2:46 p.m., deputies responded to the accident and determined that Schimek was traveling southbound on South Dayton Silver Creek Road when she lost control of her vehicle and traveled onto the shoulder of the road. The vehicle then struck a large tree, rolled onto its side and came to rest against a barn. Schimek had non life-threatening injuries.
POMFRET-Jennica Hawley, 21, of Dewittville was charged Saturday with driving while intoxicated. At approximately 3:08 a.m., deputies observed a vehicle stopped on the side of Fredonia Stockton Road in Pomfret. Upon investigation, it was determined that the driver-Hawley-was allegedly intoxicated. Hawley refused to take a chemical breath test and was arraigned in Pomfret Court. She will appear in Pomfret Court at a later date.
POMFRET – Michael Foster, 49, of Cassadaga was charged with felony driving while intoxicated, felony aggravated unlicensed operation, refusal of breath test and failure to dim high beams on Saturday at 2:36 a.m. Chautauqua County Sheriff’s deputies stopped Foster for allegedly turning on his high beams while approaching an oncoming vehicle. Further investigation found Foster to be intoxicated, and due to his driving record, his crimes were a felony. He was arraigned in Pomfret Court and was released on his own recognizance.
LAKEWOOD-BUSTI POLICE DEPARTMENT
Denae E. Doner, 32, and Thomas E. Dalton, 35, both of Jamestown were charged Sunday with petit larceny. At approximately 3:03 p.m., it is alleged that Doner and Dalton entered Wal-Mart together and began browsing the store. Doner brought two empty bags into the store and began filling them with miscellaneous items. Dalton used his jacket to hide some of the items inside the cart. He then left the store after purchasing cigarettes. Doner continued shopping at the “Book Nook” which is at the front of the store. Wal-Mart Loss Prevention observed her previous activity and took her to their office. Police located Dalton sitting in his vehicle in the parking lot. Both will appear in Busti Court at a later date.
CATTARAUGUS COUNTY SHERIFF’S OFFICE
KILLBUCK-Samantha J. Miller, 26, of Salamanca was charged Sunday with third-degree criminal trespass. At approximately 2:30 a.m., deputies received a report of an unruly patron who refused to leave after last call. It is alleged that the patron-Miller-became physically aggressive when an employee attempted to escort her. Miller was arraigned in Great Valley Court and remanded to Cattaraugus County Jail in lieu of $250 bail. She is scheduled to appear in Great Valley Court on Jan. 8.
SALAMANCA – Juanita Rivera, 36, of Salamanca was arrested at her residence on a bench warrant issue from Cattaraugus County Court on Dec. 19 at 7 p.m. She was transported to the Cattaraugus County Jail and held in lieu of bail.
JAMESTOWN POLICE DEPARTMENT
Kevin Diaz, 19, of Jamestown was charged with second-degree criminal contempt and a bench warrant Friday at 11:09 p.m. Jamestown officers were dispatched to East Second Street for a possible underage party. Officers didn’t find a party, but did locate Diaz, who officers knew to have a valid bench warrant from Jamestown City Court. Further investigation revealed Diaz allegedly had a valid order of protect against a resident who lived at the address. He was transported to the Jamestown City Jail where he was awaiting arraignment.
Thomas DiDomenico, 29, of Jamestown was charged with driving while intoxicated, unsafe start, failure to keep right and refusal to take a pre-screen test on Saturday at 2:29 a.m. Jamestown Police officers stopped DiDomenico on Broadhead Avenue for unsafe start and failure to keep right, and upon further investigation, he was allegedly found to be intoxicated. DiDomenico was taken into custody and transported to Jamestown City Jail without incident. He refused to take a chemical breath test and was awaiting arraignment by Jamestown City Court.

CHAUTAUQUA COUNTY SHERIFF’S OFFICE
Daniel P. Hido, 25, of Girard, Pa., was charged Tuesday with driving while intoxicated, aggravated DWI with a blood alcohol content of .18 percent or greater and moving from lane unsafely. At approximately 7:04 p.m., deputies responded to a vehicle off the road on I-86 westbound. Upon locating the vehicle, deputies determined that Hido was the driver and allegedly found intoxicated. Hido was taken into custody and later released with uniform traffic tickets. Hido will appear in Sherman Court at a later date.
DUNKIRK – Mark Petz, 30, of Dunkirk was charged Thursday with driving while intoxicated, aggravated DWI, moving from lane unsafely and leaving the scene of a property damage accident. At approximately 2:16 a.m., deputies responded to a report of a vehicle off the roadway and into a tree. Upon arrival, deputies learned the driver had left the scene of the accident. Petz was eventually found intoxicated and taken into custody by deputies. He was issued appearance tickets.

CHAUTAUQUA COUNTY SHERIFF’S OFFICE
BUSTI-Angel L. Ormsby, 37, of Jamestown was charged Saturday with driving while intoxicated, aggravated DWI with a child less than 16 in the vehicle, driving with a blood alcohol content of .08 percent or greater, having a backseat passenger age 4-7 with no or improper restraint and speeding in a 55 mph zone. At approximately 3:15 p.m., deputies stopped a vehicle on Busti-Stillwater Road for speeding. Upon investigation, it was determined that Ormsby was intoxicated. Deputies also observed that Ormsby had a backseat passenger under 16 at the time of the stop. Ormsby was arraigned in Busti Court and transported to Chautauqua County Jail. He will appear back in Busti Court to answer for his charges.
SILVER CREEK-David Sodaro, 55, of Silver Creek was charged Friday with fourth-degree criminal mischief and second-degree menacing. At approximately 7:07 p.m., Sodaro allegedly intentionally damaged property by using his body to break a front door window. Sodaro then allegedly pulled out a large pocket knife and started waving it in a threatening manner at his girlfriend. Sodaro threatened his girlfriend further by allegedly throwing the knife and sticking it in the floor. Sodaro was remanded to Chautauqua County Jail in lieu of bail.
STOCKTON-Jason E. Perittie, 26, of Jamestown was charged Friday with second-degree burglary and fourth-degree grand larceny. At approximately 8:40 p.m., deputies received a complaint of a suspicious vehicle parked in front of a seasonal residence on Barnes Road. Upon arrival, deputies and Sheriff K9 unit located household items that had been removed from the residence. Sheriff’s K9 Drago was deployed into a wooded area in an attempt to track the alleged burglar. After a lengthy trail, Perittie was apprehended.
CHAUTAUQUA-Dominick Luciano, 30, of Jamestown was charged with felony driving while intoxicated, aggravated DWI per se and speeding.At approximately 11:50 p.m., Luciano was stopped for allegedly speeding in excess of 80 mph on Route 394. Further investigation found Luciano to be intoxicated. Luciano had a previous intoxication in the previous 10 years, making this arrest a felony.
DUNKIRK-Ruth A. Santiago, 30, of Dunkirk was charged Thursday with third-degree welfare fraud, two counts of offering a false instrument and misuse of food stamps. Ricardo Negron-Garcia, 26, of Dunkirk was also charged Thursday with third-degree welfare fraud and misuse of food stamps. It was alleged that from Nov.1, 2012 through Feb. 28, 2013, Santiago failed to report that her husband, Garcia, was employed while the parties were residing together. This unreported income allowed them both to receive $4,110.60 in unentitled benefits. Both are scheduled to appear in Jamestown Court at a later date.
ELLICOTT POLICE DEPARTMENT
Tammy J. Lore, 36, and Cheyenne J. Lore, 18, both of Randolph was charged Monday with petit larceny. According to police, both subjects were inside K-Mart on Fairmount Avenue when they were found removing items from the store without paying for them. They will appear in Ellicott at a later date.
Josh A. Moore, 33, of Mayville was charged Friday with fourth-degree grand larceny, endangering the welfare of a child and being a fugitive from justice. Police investigated a report of a subject stealing from a local business. Through this investigation, a vehicle description and plate was obtained. Police located the vehicle at a local scrap yard and took Moore into custody on an active bench warrant. Further investigation revealed that Moore had taken $1,085.77 worth of merchandise from Home Depot. Moore also had a warrant out of Pennsylvania leading to the fugitive from justice charge. Moore was remanded to Chautauqua County Jail.
Caleb M. Wickstrom, 23, of Falconer was charged Monday with false personation. Wickstrom had an arrest warrant issued for violation of probation, where it is alleged he had neglected agreed terms of his conditions of probation. Wickstrom turned himself into police. He was remanded to Chautauqua County jail and will be back in Ellicott Court at a later date.
Josh M. Verrett, 16, of Jamestown was charged Tuesday with false personation. While checking a subject walking on West Falconer Street at 3 a.m., it was found that Verrett had falsely identified himself. He was issued an appearance ticket and released to a relative.

CHAUTAUQUA COUNTY SHERIFF’S OFFICE
SILVER CREEK-Joshua C. Stull, 37, of Silver Creek was charged Monday with driving while intoxicated per se, DWI, aggravated DWI with a blood alcohol content of .18 percent or higher and speeding. At approximately 12:13 a.m., deputies observed a vehicle traveling at a high rate of speed over the posted limit. Upon stopping the vehicle, deputies determined that Stull was intoxicated. Stull was taken into custody. He will appear in court at a later date.
POMFRET- Christopher Parker, 28, of Wattsburg, Pa. was charged with fourth-degree criminal possession of marijuana, third-degree aggravated unlicensed operation, inadequate muffler and no right side mirror on Friday at 11:16 p.m. Chautauqua County Sheriff’s deputies observed Parker traveling on Route 20 in Pomfret with a discernibly loud and leaking exhaust and no right sideview mirror. Allegedly, Parker was in possession of over 2 ounces of marijuana and multiple smoking pipes, and an investigation revealed that his New York driving privileges were suspended. He was issued citations and appearance tickets, and will appear in Pomfret Court at a later date.
BROCTON- Peter Galardo, 32, of Fredonia was charged with aggravated driving while intoxicated with a blood alcohol content of .18 percent or higher, DWI, DWI with a blood alcohol content of .08 percent or higher, third-degree aggravated unlicensed operator, failure to keep right and no/inadequate tail lamps on Saturday at 1:46 a.m. Chautauqua County Sheriff’s deputies allegedly observed a vehicle failing to maintain its lane and having inadequate tail lamps while traveling in Brocton. Galardo’s driving privileges were suspended and he was intoxicated, police said. He will appear in Pomfret Court at a later date.
PORTLAND- Andrew Davidson, 39, of Westfield was charged with a felony driving while intoxicated, unreasonable speed, refusal to submit to a breath test and driving out of restrictions on Friday at 1:48 p.m. Chautauqua County Sheriff’s deputies responded to a one-car accident on Prospect-Station Road at the intersection of Finley Road. Deputies determined Davidson was traveling at an unsafe speed and failed to make a right hand turn onto Finley Road from Prospect-Station Road. The vehicle crossed over Finley Road and hit a snow bank before crashing into trees. Davidson was allegedly intoxicated, and an investigation revealed two prior convictions for driving while intoxicated, making the arrest a felony, police said.
CHAUTAUQUA- Joshua Powell, 28, was charged with second-degree burglary, fifth-degree possession of stolen property and seventh-degree criminal possession of a controlled substance on Saturday at 7:30 a.m. Chautauqua County Sheriff’s deputies responded to a residence in Mayville for a possible burglary in progress. Before police arrived, Powell allegedly fled the scene. Deputies were able to locate him behind a business in Mayville, and upon investigation, allegedly Powell did not have permission to be in the dwelling and attempted to remove multiple items from the residence. After placing Powell in custody, he was also found to be in the possession of a controlled substance, police said. He was arraigned and is in the custody of Chautauqua County Jail.
CHAUTAUQUA- Joseph Spas, 55, of Ashville was charged with driving while intoxicated, aggravated DWI with a blood alcohol content of .18 percent or greater and failure to keep right on Saturday at 8:57 p.m. Chautauqua County Sheriff’s deputies stopped Spas on Route 394 in Chautauqua for failing to keep right, and allegedly Spas was intoxicated. He was taken into custody and later released with uniform traffic tickets. Spas will appear in Chautauqua Court at a later date to answer the charges.
JAMESTOWN POLICE DEPARTMENT
James Grant, 50, of Jamestown was charged with felony driving while intoxicated, first-degree felony aggravated unlicensed operation, no/inadequate lights and operating a motor vehicle without an interlock device on Saturday at 12:10 a.m. Jamestown Police officers allegedly observed a grey Honda Civic traveling northbound on Foote Avenue without any headlights on. Officers stopped the car, and upon further investigation, Grant was allegedly found to be operating the vehicle in an intoxicated condition. Officers also learned that Grant was operating the vehicle while having a revoked driver’s license stemming from a past DWI conviction in 2012. He was arrested and transported to the Jamestown City Jail.
Alan Rowley, 30, of Jamestown was charged with felony driving while intoxicated, first-degree felony aggravated unlicensed operation and failure to use a turn signal on Saturday at 12:13 a.m. Officers allegedly observed a vehicle failing to use a turn signal while turning off of Colfax Street on to Newland Avenue. Rowley was allegedly found to be intoxicated, and an investigation revealed a previous DWI conviction in 2007 and revoked driving privileges in New York. Rowley was transported to the Jamestown City Jail for a chemical test, which he failed, police said. He was held at the Jamestown City Jail pending an arraignment.
Virginia Sanders, 31, of Sherman, was charged with a misdemeanor driving while intoxicated and a misdemeanor driving with a blood alcohol content of .08 percent or greater on Sunday at 2:37 a.m. Sanders was allegedly observed in the drive through at McDonald’s on East Second Street with open containers in her car. She was found to be intoxicated and will be arraigned in the Jamestown City Court, police said.
Jonathan Martinelli, 21, of Jamestown was charged with unnecessary noise and resisting arrest on Sunday at 1:17 a.m. Jamestown Police observed Martinelli in front of 246 Willard St. allegedly being disorderly and yelling profanities due to an earlier incident. He was allegedly advised by officers multiple times to stop yelling, which he did not do, police said. He charged at a female and officers, police said. After a brief struggle, Martinelli was taken into custody and transported to Jamestown City Jail where he was awaiting arraignment.

SHERIDAN – Christin Tibbetts, 22, of Dunkirk was charged Thursday with third-degree burglary and fourth-degree grand larceny. At approximately 4:46 p.m., Tibbetts was taken into custody as a result of an alleged incident that happened in August 2013. Tibbetts allegedly broke into a garage and stole property valued at more than $1,000. Tibbetts was transported to Chautauqua County Jail in lieu of bail.
JAMESTOWN POLICE DEPARTMENT
Bret W. Teboe, 54, of Jamestown was charged Friday with improper right-hand turn, failing to keep right, driving while intoxicated and DWI per se. At approximately 1:57 a.m., police stopped Teboe after observing him make an improper right-hand turn and failing to keep right. Police determined Teboe to be intoxicated. He was transported to Jamestown City Jail.
Joseph M. Dominick, 48, of Jamestown was charged Friday with speeding in zone and driving while intoxicated. At approximately 1:53 a.m., police observed a vehicle traveling quickly across the south side of town from Cole Avenue to City View Avenue. The vehicle was found to be traveling north on City View Avenue at 50 mph. The vehicle was stopped on Hallock Street at Hillcrest Avenue. Dominick was found to be intoxicated. He will be arraigned in the Jamestown City Court.

Stacey A. Scinta, 23, of Cherry Creek was charged Thursday with petit larceny. After police were called to a shoplifting complaint at K-Mart on Fairmount Avenue, it was determined that Scinta had taken various property from the store and exited without paying for it. Scinta will appear in court at a later date.
LAKEWOOD-BUSTI POLICE DEPARTMENT
Justin M. Kinney of Randolph was charged Thursday with driving while intoxicated per se, DWI .08 percent or higher and speeding in zone. At approximately 8:56 p.m., police observed a vehicle speeding on Hunt Road in Busti. After investigation, it was determined that Kinney was operating the vehicle intoxicated. He will appear in Busti Court at a later date.
CATTARAUGUS COUNTY SHERIFF’S OFFICE
Nathan J. Ellis, 32, of Salamanca was charged Friday with stopping/standing/parking on highway, consumption of alcoholic beverage/possession of open container, driving while intoxicated, aggravated DWI and refusal of breath screening test. At approximately 12:30 a.m., Ellis was found on Bunker Hill Road in Coldspring. Ellis was arraigned in Randolph Court and remanded to Cattaraugus County Jail on $5,000 cash bail. He will appear in Coldspring Court on Jan. 14.

Nicole M. Esner, 41, of Irving was charged Thursday with unlawful possession of marijuana. At approximately 3:15 a.m., deputies were dispatched to 1049 Shady Drive to assist the fire department on an EMS call. Upon arrival, deputies observed a clear baggie containing marijuana in plain view. Esner will appear in Hanover Court at a later date.
JAMESTOWN POLICE DEPARTMENT
Jose A. Jimenez, 26, was charged Wednesday with theft of services. At approximately 7:02 p.m., officers were dispatched to 228 Prospect St. for a male refusing to pay for a taxi cab ride. Upon arrival, officers made contact with the Rainbow Taxi driver and Jimenez. Officers determined that Jimenez owed the taxi company money for transportation and Jimenez would not pay the fare. Jimenez was transported to Jamestown City Jail.

Nichole Verrett, 26, of Jamestown was charged Wednesday with petit larceny. At approximately 12 p.m., officers were dispatched to Big Lots on 39 S. Main St. for a shoplifting incident. Upon arrival, officers determined that Verrett was the suspect involved in the shoplifting incident. Verrett was transported to Jamestown City Jail.
CATTARAUGUS COUNTY SHERIFF’S OFFICE
Alan B. Dowdy, 22, of Great Valley was charged Dec. 11 with leaving the scene of a motor vehicle crash, driving without a license, failure to keep right and aggravated driving while intoxicated. At approximately 4:55 a.m., deputies were called to the scene of a motor vehicle crash on Route 417. Upon investigation, it was determined that Dowdy was the operator. He was not injured. Dowdy will appear in Great Valley Court at a later date.
Jordan D. Spry, 24, of Randolph was charged Monday with violation of probation. At approximately 4:23 p.m., deputies took Spry into custody for violating probation. He was remanded to Cattaraugus County Jail.

- Donald L. Sturdevant, 57, of Warren, Pa., was charged Tuesday with driving while intoxicated. At approximately 8:04 p.m., officers determined Sturdevant was intoxicated after he made a right turn on a red light, where it was posted “No Turn on Red.”

Richard Kniese, 55, of Dunkirk was charged Tuesday with second-degree aggravated harassment. At approximately 2 p.m., deputies charged Kniese after an investigation determined that he had allegedly called a business on Dec. 5 and left an offensive message on the answering machine. Kniese will appear in Silver Creek Court on a later date.

SILVER CREEK-Ralph Crawford, 47, of Silver Creek was charged Sunday with driving while intoxicated, DWI per se and moving from lane unsafely. At approximately 7:06 p.m., deputies observed a vehicle that was headed eastbound on Central Avenue veer off the road and into a ditch. Further investigation determined that the driver- Crawford- was intoxicated. Crawford will appear in Silver Creek Court at a later date.
HANOVER-An unidentified 17-year-old male of Forestville was charged Sunday with driving across hazard markings, driving while intoxicated and DWI with blood alcohol content of .08 percent of 1 percent or more. At approximately 3:04 a.m., deputies were dispatched to Bradigan Road for an alleged vehicle off the road. Upon arrival, deputies observed a pickup truck off the road which had struck a utility pole. After investigation, it was determined the driver-the 17-year-old male-was intoxicated. The male will appear in Hanover Court at a later date.

Lynn A. Lee, 51, of Falconer was charged Friday with disorderly conduct. Shortly before midnight, police were dispatched to the Budget Inn in Falconer for a report of a disorderly subject. Upon arrival, police observed Lee disorderly and creating unnecessary noise by yelling obscenities and banging on doors. Lee was transported to Chautauqua County Jail in lieu of $500/$1,000 bail. She will appear in Ellicott Court on a later date.

Two unidentified males – one 16-year-old and one 17-year-old – were charged Saturday with unlawful possession of marijuana and underage possession of alcohol. At approximately 10:36 p.m., police responded to 522 Pine Ridge Road, where an underage drinking party was taking place. Officers observed the above subjects in possession of alcohol and marijuana. Both will appear in Busti Court at a later date.

LAKEWOOD-BUSTI POLICE DEPARTMENT
Andrew C. Carlson of Bemus Point was charged Thursday with driving while intoxicated, following too close and third-degree aggravated unlicensed operation. At approximately 2:15 p.m., Carlson was taken into custody after being involved in an accident on Fairmount Avenue in Lakewood, police said. Carlson was found to be intoxicated, police said. He will appear in Busti Court on a later date.
Shane R. Eshbach, 23, of Jamestown, was charged Thursday with fifth-degree conspiracy. Investigation determined that Eshbach had conspired with a juvenile to commit petit larceny at Wal-Mart in Lakewood.
Dwain Graham, 19, and Jamie Hohmann, 18, both of Randolph, were charged Thursday with petit larceny. At approximately 10 p.m., Graham and Hohmann were caught by Wal-Mart staff shoplifting merchandise, police said. They will appear in Busti Court on a later date.

Barry A. Hannold, 62, of Lakewood, was charged Thursday with driving while intoxicated, aggravated DWI, failing to keep right and leaving the scene of a personal injury accident. At approximately 7:08 p.m., police were called to a hit and run accident at the corner of Hunt Road and Robinson Avenue. Hannold was found to be the operator of the suspect vehicle, police said. He was intoxicated, police said. He will appear in Ellicott Court on a later date.
Walter M. Westerdahl, 32, of Falconer, was charged Dec. 5 with second-degree burglary and fourth-degree grand larceny. After an investigation into a burglary that occurred at a Falconer residence, police alleged that Westerdahl was the one who entered the residence while the homeowners were at work and stole $3,985 in cash. He was sent to Chautauqua County Jail.

Justin D. Miller, 32, of Frewsburg, was charged Thursday with operating a motor vehicle while using a mobile phone, driving while intoxicated and driving with a blood alcohol content of .08 percent or more. At approximately 11:30 p.m., officers responded to a motor vehicle accident on South Main Street at Newland Avenue, where a parked vehicle was struck. Upon arrival, officers determined that the suspect driver was Miller, who had two warrants out of Jamestown, police said. Miller was found to be intoxicated at the police department. He will be arraigned in Jamestown Court.
Richard E. Pearson, 60, of Jamestown, was charged Friday with no/improper signal, driving while intoxicated and driving while intoxicated per se. At approximately 12:05 a.m., police stopped Pearson for an alleged traffic violation at Winsor Street and East Second Street. Investigation determined that Pearson was intoxicated, police said. After being transported to the city jail, Pearson took a chemical breath test and had a blood alcohol content of .08 percent or more, police said. Pearson is in the city jail awaiting arraignment.

Roy Lobell, 74, of Westfield was charged Tuesday with driving while intoxicated, DWI per se and failure to keep right following a motor-vehicle accident. Deputies responded to a report of an accident on Center Road in Town of Sheridan just after 7:30 p.m. Lobell’s vehicle had been traveling southbound before it left the roadway, coming to a rest on its side in a ditch located on the east side of the road. LAKEWOOD-Chad M. Whittington, 33, of Lakewood was charged Wednesday with petit larceny, fraudulent receipt of benefits and misuse of food stamps. According to police, it is alleged that Whittington failed to report employment/income to the Chautauqua County Department of Social Services. Whittington received $1089.68 in unentitled Medicaid and food stamp benefits as a result of this unreported income. Whittington will appear in Jamestown City Court at a later date.

Ralph Pratt, 49, of Jamestown was charged Thursday with two counts of petit larceny and two counts of fourth-degree grand larceny. Pratt was transported to Chautauqua County Jail.

Paul D. Surrena, 46, of Jamestown, was charged Saturday with first-degree felony aggravated unlicensed operator, second-degree criminal impersonation, driving while ability impaired, unlicensed operator and unregistered vehicle. At approximately 2:45 a.m., a patrol stopped an unregistered vehicle at Martin Road and Foote Avenue in Jamestown. Surrena was taken into custody for being intoxicated, police said. After taking a chemical blood test, Surrena told police that he provided a false name and date of birth, police said. Surrena was also allegedly found to have a revoked driver’s license, stemming from a previous DWI in 2000. Surrena will appear at Jamestown City Court at a later date.
JAMESTOWN POLICE DEPARTMENT
A 17-year-old female was charged Tuesday with fourth-degree criminal mischief. At approximately 7:05 p.m., officers dispatched to 200 Gustavus Ave. for a criminal mischief complaint. Upon arrival, officers were told by several personnel of the residence that the female-who is a resident of the facility-tore a phone off the wall and threw it to the ground. The female was transported to Jamestown City Jail without incident.

Debra J. Faison, 58, of Dunkirk was charged Monday with third-degree criminal possession of a controlled substance and third-degree criminal sale of a controlled substance. At approximately 8:19 p.m., police took Faison into custody on an outstanding warrant issued by the Dunkirk City Court. The warrant was issued after the task force brought information to the court that Faison had sold cocaine to agents of the task force on numerous occasions in Dunkirk, police said. Faison was transported to Dunkirk City Jail. The arrest culminates a several-months-long investigation. As a result of this investigation, additional information has been obtained, and additional investigations are anticipated.
LAKEWOOD-BUSTI POLICE DEPARTMENT
Christie A. Eddy, 41, of Jamestown, was charged on Nov. 25 for petit larceny and trespassing. At approximately 1:02 p.m., Eddy was involved in a larceny at Wal-Mart after she was advised not to enter any Wal-Mart stores in the country because of previous larcenies, police said. Eddy will appear in Busti Court at a later date.

LITTLE VALLEY-Gordon M. Sullivan, 35, of Salamanca was charged Saturday with driving while intoxicated, DWI with blood alcohol content of .08 percent or higher, leaving the scene of property damage and failure to maintain lane. At approximately 2:11 a.m., deputies were called to the scene of an accident on Route 353, where an empty vehicle was found, having struck a bridge guardrail. Sullivan was found shortly after at his residence and transported to Salamanca Police Department for further investigation. Sullivan was taken into custody after failing field sobriety tests. He will appear in court at a later date.
LITTLE VALLEY-Donald N. Michener, 51, of Little Valley was charged Saturday with criminal possession of a weapon and second-degree criminal contempt. At approximately 5:30 p.m., deputies charged Michener on a warrant issued by Little Valley. Michener is accused of violating an order of protection. He was sent to Cattaraugus County Jail in lieu of $1,000 bail.

RIPLEY- Cody Rater, 18, of Ripley was charged with unlawful possession of marijuana on Saturday at 9:41 p.m. Chautauqua County Sheriff’s deputies stopped a vehicle on Lakeview Avenue in Ripley due to the vehicle registration not matching its license plates. Rater was allegedly found to be in possession of multiple smoking pipes and a quantity of marijuana. He will appear in Ripley Court at a later date.
MINA- Taylor Bennett, 25, of Erie, Pa., was charged with driving while intoxicated, moving from lane unsafely and leaving the scene of a property damage accident on Friday at 2:22 a.m. Chautauqua County Sheriff’s deputies responded to a report of a vehicle off the road on Interstate 86 in Mina. Upon arrival, Bennett had left the scene. Deputies located him at the Holiday Inn Express a short distance away, and after an investigation it was determined Bennett was enroute to Erie from Jamestown when he lost control on the snow covered roadway. Bennett struck a highway sign, causing damage to it. Although he had the opportunity to call police to report the accident once at the hotel, he allegedly failed to do so and called for a tow truck. Bennett was taken into custody and he allegedly refused a chemical breath test, which will result in the immediate suspension of his operating privilege in New York. Bennett will appear in Mina Court to answer the charges.
PORTLAND- Karlene Taylor, 76, of Brocton was charged with driving while intoxicated, driving while intoxicated with a blood alcohol content of .08 percent or higher and failure to stop at a stop sign on Saturday at 5:35 p.m. Chautauqua County Sheriff’s deputies responded to a motor vehicle accident on Route 5 in Portland. Upon arrival and investigation, Taylor was traveling north on Lake Avenue and failed to stop at the stop sign at Lake Avenue and Route 5, police said. She traveled through the intersection on to Route 5, and into the ditch on the north side. Taylor was also allegedly found to be intoxicated. She was taken into custody where she was released and will appear in Portland Court at a later time and place.
HANOVER- Leta Halftown, 43, of Perrysburg was charged with endangering the welfare of a child and disorderly conduct on Saturday at 2 a.m. Chautauqua County Sheriff’s deputies responded to a report of a disorderly subject at the Lighthouse Inn in Hanover. Upon arrival, deputies located Halftown, who allegedly became disorderly inside a motel room, throwing items and causing damage. During the course of this act, there was a young child also present in the room, police said. Halftown was arrested and taken into custody. She was later released with appearance tickets for the charges in Hanover Court.
GERRY- Theodore Daughenbaugh, 50, of Sinclairville was charged with second-degree harassment on Friday at 10:10 p.m. Chautauqua County Sheriff’s deputies responded to Old Chautauqua Road in Gerry for a report of an altercation between two people. Allegedly Daughenbaugh had struck another person in the head with an alarm clock during the altercation. He was issued an appearance ticket and will answer the charge in Gerry Court.
HANOVER- Richard Charboneau, 38, of Irving was charged with endangering the welfare of a child and criminal obstruction of breathing or blood circulation on Saturday at 10:50 p.m. Chautauqua County Sheriff’s deputies responded to a domestic incident on Anne Street in Hanover. Upon arrival, witnesses alleged that Charboneau had choked a female subject and had done so in the presence of a small child. He was taken into custody and remanded to Chautauqua County Jail in lieu of $1,000 cash bail. Charboneau will appear back in Hanover Court at a later date.
LAKEWOOD-BUSTI POLICE DEPARTMENT
Bradley S. Baum, 37, of Sherman was charged Saturday with driving while intoxicated, driving with a blood alcohol level of .08 percent or higher, failure to keep right, operating a motor vehicle with a suspended driver’s license, operating a motor vehicle with a suspended registration, possession of an open container in a motor vehicle upon a public highway and operating an uninsured motor vehicle at approximately 12:45 a.m. Police observed Baum crossing from the southbound lane into the northbound lane before he was pulled over. Upon stopping Baum, police determined he was intoxicated. Baum also had a suspended driver’s license and registration. Baum will appear in court at a later date.
Tyler N. Chase of Lakewood was charged Sunday with aggravated driving while intoxicated with child in vehicle (Leandra’s Law), DWI, DWI per se, littering, failure to signal turn, open container inside vehicle, driving across hazard markings and two counts of endangering the welfare of a child at approximately 12:12 a.m. On Busti Stillwater Road in Busti, police observed a vehicle failing to signal a turn, driving across hazard markings and littering on the roadway. Investigation determined that Chase was intoxicated while having two 15-year-old occupants inside. Chase’s blood alcohol level was found to be twice the legal limit to operate a motor vehicle in New York state. Chase will appear in the Busti court at a later time.
Valerie L. Houser, 29, of Busti was charged Monday with unlawful imprisonment, two counts of endangering the welfare of a child and resisting arrest. At approximately 2:09 a.m., police responded to 1501 Wellman Road for a domestic dispute. Officers placed Houser in custody for refusing to let her boyfriend leave the bedroom. Houser resisted arrest. This incident took place in front of two small children. Houser was remanded to Chautauqua County Jail.
ELLICOTT POLICE DEPARTMENT
Charles A. Guiley, 35, of Clarendon, Pa., was charged Thursday with driving while intoxicated, third-degree aggravated unlicensed operator, failure to maintain lane and moving from lane unsafely. At approximately 2 a.m., police observed a vehicle on Southwestern Drive violating numerous vehicle and traffic law violations. Investigation revealed that Guiley was intoxicated and had a suspended driver’s license. Guiley was remanded to Chautauqua County Jail and will appear in court on a later date.
Holden D. Rosengren, 18, of Jamestown was charged Thursday with second-degree harassment. It is alleged that Rosengren punched another subject twice in the face on Dec. 5. Rosengren will appear in Ellicott Court on a later date.
Danielle A. Kum, 26, of Jamestown was charged Dec. 2 with petit larceny. Officers responded to a shoplifting incident at the K-Mart store on Fairmount Avenue. K-Mart officials had Kum in custody when officers arrived. Kum allegedly attempted to leave the store without paying for an item. Kum will appear in Ellicott Court on a later date.
Bonnie S. Patterson, 38, of Jamestown was charged Friday with second-degree possession of a forged instrument. Patterson was taken into custody on an arrest warrant, stemming from an incident on Oct. 18 where she allegedly cashed a forged check knowingly. Patterson was remanded to Chautauqua County Jail and will appear in Ellicott Court on a later date.

Robert Pearson II, 40, of Jamestown was charged with petit larceny and second-degree harassment on Saturday at 3:45 p.m. Jamestown Police officers responded to the Priscilla Apartments on East Fifth Street for a reported dispute. Officers spoke with two victims who said they were involved with an altercation with Pearson. Allegedly during the altercation, Pearson repeatedly pushed one of the victims and stole two plates of food from the other victim, police said. Pearson was located in the building and taken into custody without incident. He was transported to the city jail.
Robert Storms, 57, was charged with driving while intoxicated, driving while intoxicated with a blood alcohol content of .08 percent or higher and unlawful possession of marijuana on Friday at 7:46 p.m. Jamestown Police stopped Storms on Jones and Gifford Avenue in Jamestown for a traffic violation, and allegedly he was found to be driving his motor vehicle while intoxicated. Further investigation led Jamestown Police and the K-9 unit to find Storms to be in possession of marijuana, police said. He was transported to the city jail.
Adam Pavlock, 22, of Jamestown, was charged with no front license plate, driving while intoxicated and aggravated driving while intoxicated on Saturday at 1:04 a.m. Jamestown Police stopped Pavlock at East Sixth Street at Spring Street in Jamestown, and found him to be allegedly operating his vehicle in an intoxicated condition. He was transported to the city jail where he was awaiting arraignment.
Kelly Cotter, 43, of Jamestown, was charged with failure to keep right, improper right turn at intersection, misdemeanor driving while intoxicated and misdemeanor aggravated driving while intoxicated on Saturday at 12:44 a.m. Jamestown Police allegedly observed Cotter failing to keep right on Lakeview Avenue near East Eighth Street. She allegedly improperly turned onto Falconer Street and was stopped on Sturges Street near Van Buren. Allegedly Cotter was intoxicated, and was arrested and was arraigned in Jamestown City Court.
Derrick Harper, 28, of Jamestown, was charged with violation of restricted license, misdemeanor driving while intoxicated and misdemeanor driving with an alcohol content of .08 percent or greater on Saturday at 11:21 p.m. Off duty officers located a vehicle sitting in the middle of the roadway on Spruce Street at Lakeview. The vehicle allegedly had damage to it, and the driver and passenger were in the vehicle. On duty officers arrived and found Harper to be intoxicated, police said. He will be arraigned in Jamestown City Court, and the investigation for the damage to the vehicle is on-going and further charges might be pursued.

RANDOLPH- Peter Shaw-Brougham, 19, of Jamestown, was charged with resisting arrest by Jamestown Police Department on an arrest warrant issued by Randolph charging him with two counts of petit larceny and two counts of endangering the welfare of a child from an incident in early November, on Thursday at 10 p.m. He was turned over to the Cattaraugus County Sheriff’s Office and taken to Randolph Court for arraignment. Shaw-Brougham was remanded to the Cattaraugus County Jail in lieu of $2,500 bail. He is due back in court at a later date.
RANDOLPH- Dale Wagatha Jr., 31, of Little Valley, was arrested on a bench warrant issued from Randolph after being released from Chautauqua County Jail on unrelated charges on Wednesday at 10 p.m. He was taken to Randolph for arraignment and remanded to the Cattaraugus County Jail in lieu of $2,500 bail. Wagatha is due back in court at a later date.

Andrew F. Mealy, 23, of Jamestown was charged Thursday with criminal obstruction of breathing or blood circulation. At approximately 5:22 p.m., officers responded to 438 Winsor St. for a domestic incident. Officers spoke with a female victim who stated that she was allegedly choked by Mealy during a verbal argument. Officers located Mealy in the area and he was taken into custody without incident. Mealy was transported to Jamestown City Jail.
Brandon L. Calkins, 27, of Jamestown was charged Friday with improper signal and two misdemeanors – driving while intoxicated and aggravated DWI with a blood alcohol content of .08 percent or higher. At approximately 2:37 a.m., a vehicle was observed failing to signal its turn off Foote Avenue onto Prather Avenue. After the vehicle was stopped, Calkins was observed operating the vehicle intoxicated. Calkins will be arraigned in Jamestown City Court.

TIDIOUTE, Pa. – A 24-year-old Bemus Point man and a 22-year-old Tidioute, Pa. woman were both charged with harassment Thursday at 8:15 p.m. Police say the two got into an altercation over property. The man proceeded to shove the woman with both hands when the woman began to punch and kick the man. Their names were not released.

GERRY – Wigberto Torres Rodriguez, 61, of Dunkirk was charged Nov. 30 for driving while intoxicated, DWI per se, failing to keep right, crossing the hazard line and driving on the shoulder. At approximately 11:36 p.m., deputies observed a vehicle committing multiple traffic violations while traveling southbound on Route 60 in Gerry. Upon investigation, it was determined that Torres was intoxicated. Torres will appear in Gerry Court at a later date.
Roger J. Prince-Rivera, 32, of Silver Creek was charged Wednesday with criminal construction of breathing or blood circulation, fourth-degree criminal mischief, second-degree unlawful imprisonment and second-degree harassment. At approximately 4:06 p.m., deputies responded to a domestic dispute on Monroe Street in Silver Creek. Upon investigation, it was determined that Prince-Rivera hit the hand of a female victim during a domestic dispute and obstructed her from placing a phone call to emergency services, police said. It was also determined that Prince-Rivera restrained the female victim from leaving the residence and pinned the victim against a door with his hand on the victim’s throat, police said. Prince-Rivera was committed to Chautauqua County Jail on bail.

Sean M. Carlson, 29, of Ashville was charged Wednesday with third-degree criminal mischief, endangering the welfare of a child and second-degree harassment. At approximately 7:15 p.m., Carlson turned himself into police on outstanding warrants for the charges. The charges were filed following an incident where Carlson allegedly broke items with a young child nearby during an argument with a female, police said. Carlson was arraigned in Busti Court where he was released to the supervision of probation.

James A. Phelps, 39, of Kenmore, was charged Tuesday with driving while intoxicated and DWI with blood alcohol content of .08 percent or higher. At approximately 5:28 a.m., Phelps was operating a vehicle southbound on Route 219 when deputies received complaints of erratic driving, police said. After Phelps was stopped, it was determined that he was intoxicated. Phelps will appear in Salamanca Court at a later date.

Ivan J. Fullington of Jamestown was charged Tuesday with petit larceny, resisting arrest and unlawful possession of marijuana. At approximately 9:18 p.m., officers responded to Tops Market on 2000 Washington St. for a disorderly person. Officers were told by employees that a male had left the store and had stolen beer. Officers located Fullington in the Dollar Tree store. Fullington became combative as officers tried placing him into custody. Fullington was found to be in possession of a small amount of marijuana. He was transported to Jamestown City Jail where he is awaiting arraignment.

Michael L. Hitchcock, 18, of Olean was charged Friday with second-degree criminal possession of marijuana. At approximately 10:47 p.m., deputies observed Hitchcock spinning his tires as he made his turn on Humphrey Road and proceeded in the opposite direction. Investigation revealed that Hitchcock was in possession of a quantity of marijuana. He was taken for arraignment where bail was set at $5,000.
Julian G. Nelson, 27, of Leland, N.C., was charged Thursday with driving while intoxicated, aggravated DWI, unlicensed operation and no lights. At approximately 2:15 a.m., deputies conducted a routine traffic stop in Salamanca where Nelson was determined to be intoxicated. Nelson was released to a third party and will appear in Salamanca Court at a later date.
Melissa L. Doud, 35, of Jamestown was charged Monday with a bench warrant. At approximately 6:46 p.m., deputies took Doud into custody on a bench warrant taken out of Cheektowaga Court. Doud was turned over to Cheektowaga Police.
Derek S. Brink, 33, of Olean was charged Saturday with two counts of petit larceny following two different incidents that occurred at the Olean Wal-Mart and resisting arrest. Brink was arraigned at Allegany Court and will appear there on a later date.

Colton J. Parker, 23, of Sugar Grove, was charged Tuesday with public drunkeness and disorderly conduct. At approximately 1:43 a.m., Parker was found passed out in the back of a vehicle in a highly intoxicated state, police said. As he was being moved from the vehicle, Parker allegedly became argumentative and threatening, resulting in him being subdued. Parker was subsequently released to Warren County Probation.

James Nedwick, 54, of Stockton, was charged Sunday with failing to keep right, felony driving while intoxicated and driving while intoxicated per se with a blood alcohol content of .08 percent or higher. At approximately 9:42 p.m., deputies and the Dewittville Fire Department responded to a vehicle accident on Beech Hill Road in Chautauqua. Upon investigation, it was determined that Nedwick had crashed his vehicle in a ditch while intoxicated, police said. Nedwick had a previous conviction of driving while intoxicated within the past 10 years, police said. Nedwick will appear in Chautauqua Court at a later date.

Bethany L. Pritchard, 37, of Salamanca, was charged Monday with driving while intoxicated, driving while intoxicated per se, consumption of alcohol in a motor vehicle, inadequate stop lamps and uninspected motor vehicle. At approximately 1:54 a.m., officers observed a vehicle with inadequate stop lamps. Upon stopping the vehicle at 19 S. Main St., officers smelled a strong odor of alcohol from Pritchard. Pritchard then failed several field sobriety tests, police said. Pritchard was transported to Jamestown City Jail without incident and later released to a third party.

Nathan D. Strickland, 33, of Celoron, was charged Thursday with third-degree assault. Officers were called to an address in Celoron to investigate a past domestic dispute. It was reported that Strickland got into an altercation with a female and allegedly pushed and kicked her. The kick caused pain and bruising, police said. Strickland allegedly fled the scene prior to police arrival. Strickland was later picked up by police before being brought to Ellicott Court for arraignment, police said. He was remanded to Chautauqua County Jail and will appear at the Ellicott Court at a later date.

Raymond E. Blair, 33, of Steamburg, was charged Friday with resisting arrest and an outstanding warrant from Warren County. At approximately 11:30 p.m., deputies conducted a traffic stop in Randolph because a warrant was issued by Warren County Sheriff’s Office for Blair, police said. During the arrest, Blair allegedly attempted to resist arrest by two deputies. Blair was taken to the South Valley Court for arraignment and remanded to Cattaraugus County Jail in lieu of bail to await extradition to Pennsylvania.
Thomas R. Siperek, 56, of Little Valley, was charged Wednesday for driving while intoxicated, driving while intoxicated with a blood alcohol content over .08 percent or higher and failure to maintain lane. At approximately 9:50 p.m., Siperek’s vehicle was found in the ditch on Route 242 in Napoli, police said. Siperek was processed and released on tickets and will appear in Napoli Court on a later date.

LAKEWOOD- Christopher Powers, 28, of Randolph, and Dale Wagatha, 31, of Little Valley, were charged with petit larceny on Wednesday at 10:04 p.m. Lakewood-Busti Police responded to Wal-Mart for a report of two subjects stealing merchandise, and upon arrival, officers allegedly located Powers and Wagatha with stolen merchandise. Powers was issued an appearance ticket to appear in Busti Court at a later date. Wagatha was arraigned in Busti Court and remanded to Chautauqua County Jail due to having an outstanding warrant out of Cattaraugus County.
BUSTI- Two men face charges after a traffic stop on Mill Road in Busti. Lakewood-Busti Police officers were on patrol when an officer allegedly observed Keith Haskins, 44, of Russell, Pa., drinking an alcoholic beverage inside a vehicle and the operator, Richard Haskins, 47, of Russell Pa., failing to keep right on the roadway. Upon further investigation, Richard Haskins was allegedly found to be intoxicated. Richard Haskins was charged with driving while intoxicated and failure to keep right, and Keith Haskins was charged with possessing an open container inside a motor vehicle. Both will appear in Busti Court at a later date.

The incident started when a 31-year-old victim came home from work to find Pierce allegedly kissing Gaddy. The victim began to allegedly yell at Gaddy, who started to allegedly punch and knee the victim in the face. Pierce allegedly began to punch the victim in the back of the head multiple times. Both were arraigned and committed to Warren County Jail on $15,000 bail.
SHEFFIELD- The Sheffield Middle and High School in Sheffield, Pa., was cited for raising a false alarm for the fifth time in a 12-month period on Saturday at 7:34 a.m.

CHAUTAUQUA COUNTY SHERIFF’S OFFICE
SINCLAIRVILLE – Karen A. Reynolds, 33, of Conewango Valley, was charged early Saturday morning for driving while intoxicated. At approximately 1:07 a.m., patrol observed a vehicle make a turn without signaling on Jamestown Street in the village of Sinclairville. The vehicle was stopped, and Reynolds was found to be the vehicle’s operator, police said. Following an investigation, she was taken into custody and charged with DWI, a blood-alcohol content level of .08 percent or greater and failure to signal a turn. She will appear in Charlotte Court at a later date.
DUNKIRK – Lucy M. Warmbrodt, 23, of Dunkirk, was cited Thursday morning for driving while intoxicated. At approximately 2:40 a.m., deputies stopped a vehicle for speeding on Route 5 in the city of Dunkirk. Upon investigation, Warmbrodt was found to be intoxicated and was taken into custody before being released with traffic tickets, police said. Warmbrodt was charged with speeding, DWI and DWI per se with a blood-alcohol content of .08 percent or higher. She will appear in the city of Dunkirk Court at a later date.
CARROLL – Susan A. Burns, 62, of Rock Hill, S.C., was traveling eastbound on Route 62 in the town of Carroll when the vehicle she was driving struck a pedestrian Friday. The pedestrian, Darrell Green, was in the eastbound lane directing traffic for a U-Haul trailer that was being extracted from a ditch on the westbound side of the road. Greene was not clearly visible to Burns until the moment before the accident occurred. Greene was taken by helicopter to Hamot Hospital in Erie, Pa., to be treated for non-life-threatening injuries. An investigation into the incident is continuing.
CLYMER – Curtis W. Irion, 36, of Butler, Pa., was charged following a traffic stop Friday evening. At approximately 5:36 p.m., deputies stopped Irion for an observed violation of the vehicle and traffic law. A subsequent investigation led to the discovery of a small quantity of marijuana, and an unregistered and loaded handgun inside of the vehicle, police said. Irion was charged with second-degree criminal possession of a weapon, unlawful possession of marijuana and vehicle and traffic infractions. Irion was arraigned in the town of Sherman, and committed to the Chautauqua County Jail without bail. The town of Clymer judge was unavailable.
