New York Connects Conducting Survey

Our NY Connects program is doing a survey!

Why?

Because we want to know what it is that people in Chautauqua need. What keeps them from getting the services they need and want?

We believe that people will need more help in the future with long-term care services and support. When we age in our homes; people generally have more complex care needs. So, we are asking that everyone take a short survey if you fall into one of three categories.

1. A consumer: those receiving some kind of long-term services or support or are in need of them

2. A caregiver for an individual of any age and/or a parent of a child in Chautauqua County

3. A professional: those who work at an agency that provides care services, at every level of the agency or business.

Our county census is 126, 807 people according to the July 2021 census.

Many in our communities may need services to be started or need the services they have to continue to live in the community. Our NY Connects Long Term Care Council can help by advocating for services that are needed and for help in the workforce or funding to deliver these services to our communities. That is why we need your help. Please call or email us to get a copy of the survey that you feel best describes your situation.
