NY Connects is your trusted place to go for free, unbiased information and assistance. We can help you link to long term services and supports, such as home care, health insurance needs, transportation options, nutrition supports and many more topics that you may be needing help with.

We work closely with many organizations to connect you with the right supports: mental health, the independent living center, Medicaid and more. Our goal is to help individuals with their daily needs and services that assist them.

NY Connects serves individuals with disabilities of all ages. We can help families, caregivers, friends/neighbors, and professionals, young adults, older adults, caregivers for people including children with special care needs.

How can we help you? Anyone can get help with information about resources by telephone or online. We can help link you to long term services and supports regardless of diagnosis, age, or how you may need to pay for those services. We coordinate with other agencies to guide you through the eligibility processes for different programs and follow up with a call to see that you got the services you were needing and applying for.

We help with applications and enrollment in public assistance benefits. We can send you applications in the mail or you have the NY state website mybenefits.ny.gov is available.

You can see if you are eligible for HEAP, SNAP, health insurance, temporary assistance, and other services. Visit www.mybenefits.ny.gov to do your own prescreen anytime.
