NY Connects is staffed by specially trained operators working to understand your needs,inform you, connect you, and work with you to get you the help that you may need during different events in your life. There is a NY connects program in every county in New York State. The Chautauqua County NY Connects is a program brought to the community by the Chautauqua County department of health and human Services and the Chautauqua County Office for the Aging. There is also another community partner here in Chautauqua County that has a NY Connects program – Southwestern Independent Living Center (SILC). SILC provides services that assist people with disabilities in reaching maximum independence. They can be reached at 716-661-3010. You can reach out to either program for assistance. NY Connects is your trusted place to go for free, unbiased information and assistance. We can help you link to long term services and supports such as financial benefits, care in your home, health insurance information, transportation and nutrition or food/meals.

Our goal is to help individuals remain in their homes for as long as they can. NY Connects serves older individuals and individuals with disabilities of all ages. We can help families, caregivers, and professionals find community help. When people find us or get a chance to hear what we can help with – they always say, “I wish I had known this before”.

How can the NY Connects program help you? Information is powerful! Anyone can get help with information about resources by telephone, online, or e-mail. We can help link you to long term services and supports regardless of diagnosis, age, or payor source. We coordinate with other agencies to guide you through application and the eligibility processes. We provide seamless linkages for any necessary assessment you may need. We work with you on application and enrollment in public assistance benefits. All these things are here for you. In our experience-most people find out way too late that something or some program could heave helped them in their time of need
