It has come to our attention that an organization known as the Hospice Support Fund headquartered in Merrifield, VA has been soliciting donations by mail across Western New York. This entity is in no way associated with Chautauqua Hospice & Palliative Care and the donations that they receive will in no way benefit local residents or our organization.

CHPC would like the community to be aware that any fundraiser that is endorsed by our organization will be clearly described as such, and will display the CHPC name, logo and local contact information.

We encourage anyone wishing to donate to a specific hospice organization to look up the address and website, or call the organization directly to find out where to send money. Donations to Chautauqua Hospice and Palliative Care may be sent to 20 W. Fairmount Ave, Lakewood, NY 14750 or made directly on our website at www.chpc.care.

Chautauqua Hospice and Palliative Care is extremely thankful for the support of our community, without which we would be unable to provide the comprehensive education and care that Chautauqua County residents and their families have received for over 35 years.
